Background There are many types of minimally invasive lumbar disc surgery. These include the recently introduced biportal endoscopic lumbar disc fusion surgery. Two-port endoscopic transforaminal lumbar interbody fusion (TLIF) can combine the advantages of minimally invasive TLIF and endoscopic spinal approaches. However, clinical evidence to support double-door endoscopic TLIF remains insufficient.

Methods A comprehensive review of the English literature on biportal endoscopic lumbar interbody fusion was conducted. Articles in PubMed, the Cochrane Library and Web of Science on dual-port endoscopy TLIF use the terms “unilateral dual-port endoscopy”, “dual-port endoscopic spine surgery”, “transforaminal” and “lumbar spine” interbody fusion” and their combinations. This article analyzes and summarizes the clinical and imaging outcomes of double-door endoscopic TLIF. A dual-entry endoscopic surgical technique for lumbar interradicular fusion is then described.
Results There are 3 two-door endoscopic TLIF techniques. In the available literature, 1-year postoperative outcomes of biportal endoscopic TLIF are comparable to those of posterior lumbar interbody fusion (PLIF) and minimally invasive (MIS)-TLIF. Clinical parameters were significantly improved after double-door endoscopic TLIF. Two-door endoscopic TLIF may have the advantage of faster recovery compared to PLIF or MIS-TLIF. Two-door endoscopic TLIF did not show poor fusion rates compared with PLIF or MIS-TLIF. Postoperative complications are usually mild.
CONCLUSIONS: The clinical and imaging results at 1 year after double-portal endoscopic TLIF are better than those of PLIF and MIS-TLIF. However, long-term outcomes need to be investigated in the future through prospective randomized controlled trials.
